Pre-Admission

At Ozark Prosthetics and Orthotics we take the process and make it as easy as we can.

We do this by gathering all the necessary information such as your insurance information, your physician clinical notes and the written prescription.

You will receive paperwork to fill out when you come to your first appointment. Be sure to make a list of any questions you may have for the Practioner. Be sure to make a note of any discomfort you may have so the Practioner can address it. Have your insurance cards and bring your written prescription from your physician at your initial appointment. We do not always receive your written prescription from your physician's office.

Please wear comfortable clothing at your appointments. If you are a lower extremity amputee it is recommended you wear shorts if at all possible. Our facility is a patient focused center, if you have any concerns, don't hesitate to express those concerns or needs to us at anytime.
