Preparing for Your First Appointment
A little preparation can help you get more out of your visit to Ozark Prosthetics and Orthotics. Use this checklist before coming to our Springfield clinic for prosthetic care, orthotic bracing, or shoes and inserts.
Before You Come In
Contact our office to confirm what is needed for your appointment. Depending on your care, we may need a prescription, clinical notes from your referring provider, or other documentation.
If your provider has sent a referral or prescription, please check with us that it has arrived. Bring a copy of your prescription if you have one.
What to Bring
• Photo identification.
• Current insurance cards for each of your plans.
• Any prescription or referral you have received.
• Your current prosthesis, brace, shoes, or inserts, if applicable.
• Shoes and socks you normally wear when relevant to your fitting.
• A list of questions or concerns for your practitioner.
What to Wear
Wear comfortable clothing that allows your practitioner to examine the area being evaluated.
For a lower-limb prosthetic appointment, shorts can be helpful. For footwear appointments, bring the socks you expect to wear with your shoes or inserts.
If you’re unsure what clothing or footwear would be best, call our office before your visit.
Think About Your Goals
Consider the activities that matter most to you and any difficulties you would like to discuss. These might include getting around your home, working, enjoying a hobby, or using your current device comfortably.
Make a note of any discomfort, changes in fit, or problems with your device. Sharing these details helps your practitioner understand your needs.
Paperwork & Questions
You will have paperwork to complete at your first appointment. If you need help completing forms or have accessibility needs, let us know when scheduling so we can discuss arrangements.
